In order to qualify for treatment under the Adopt a Vet Dental Program,
a veteran must:

* Be a United States Armed Forces Veteran
* Reside in Carson City, or surrounding area (Contact us to verify your area is included)
* Have a current Veterans Administration Physician of Record and a copy of their DD-214
* Have a Net Monthly Income of Less than:
* Single: $ 1458.75
* Married/Partner: $1938.75
* Declare any Public Assistance you may be currently receiving (Medicaid, Food Stamps, etc)
* Consent to and sign AAVD’s “Veteran Intake Form”.
